Transition of Care

Transitional Care or Transition of Care refers to the transition of young people to adult based serves. As children mature into young adults, they outgrow the expertise of pediatric services and need to find an adult health service that suits them.

It is often a hard time for parents/patients to make the transition to someone new, as we may have been the only physicians and nurses the patient has known over the past 18-21 years!

Many young adults transfer to their parent’s physician. Others choose to make their own choice. Our staff can assist with providing names of local physicians and coordinating the transfer of medical information after proper consents have been signed by the patient (18 years & above). If the patient has a chronic health condition that is more involved, our staff will provide more detailed care coordination so the transition to adult medicine will be a smooth one.
