SVPAM becomes a Pennsylvania Medical Home

SVPAM works closely with the Pennsylvania American Academy of Pediatrics EPIC IC, with their Medical Home Initiative. Additionally SVPAM is nationally recognized by NCQA, The National Committee for Quality Assurance, as a Patient-Centered Medical Home.

In Pennsylvania, this program is a statewide quality improvement initiative for child and youth with special health care needs. A medical home is not a building, a house or a hospital. It is an approach to health care that emphasizes the partnership between pediatric clinicians and families. The mission is to enhance the quality of life for the patient and their families through recognition and support of families as the central caregiver for their children, effective community-based coordination, enhanced communication and improved primary health care.

SVPAM’s goals are to:

• Support quality improvement/practice transformation
• Incorporate family centered care
• Promote care coordination
• Develop data collection methods
• Ensure financial viability
• Foster high quality, efficient health care delivery

As we move through this process - we will engage PARENT PARTNERS to joy us in this journey. Having Parent Partners fosters understanding and cooperation, promotes respectful, effective partnerships, results in efficient planning to ensure services meet family needs and provides mechanisms for consumer input.
