Please note:

If you are transferring your medical care to another practice, SVPAM sends all medical records via DIRECT secure email exchange. At this point, the DIRECT email exchange is the most secure “email” exchange available so it is SVPAM policy that we utilize only this exchange for sending records to other providers. If your new provider does not participate in a DIRECT exchange, we will send securely to the parent/legal guardian via a general email platform like Yahoo/Gmail etc with your specific consent for a $6.50/fee, the amount permitted under the federal HIPAA guidelines. There is no charge for provider to provider electronic transfer. Please be advised that while this email to the parent will be secured via our secure messaging encryption, we still can’t guarantee the highest level of security like with the DIRECT exchange provider to provider since we would be utilizing a general email platform.

Per HIPAA guidelines - we will process all medical record requests within 30 days unless you are notified otherwise by our office.

We encourage you to make sure your provider is following the HIPAA regulations and utilizing secure, DIRECT email for records to be exchanged.

Also note: if you are transferring our, your portal account only remains active for 2 weeks post transfer.
